The most likely scenario is that it's a sucking chest wound suffered from shrapnel as a result of an improvised explosive device (IED). In this kind of scenario, the best treatment is to seal off the wound with a piece of plastic taped down on three of four sides — that will keep the lung inflated without letting him drown in his own blood.
